A Detailed Breakdown of 14 Risk Factors for Developing Dementia

The Lancet first published this large-scale report, prepared by an international commission of leading experts in neuroscience and geriatrics, in December 2017. Back then, it included nine risk factors for dementia.

Then, in 2020, the commission updated its previous findings and added three more factors.

When I started collecting information on this topic, that second updated version, with 12 risk factors, had just been released.

In August 2024, The Lancet “delighted” everyone with two more factors, which, by that point, no longer surprised me.

The growing list

After studying this field for several years, I understand that even back in 2017 they could have published all 14 factors at once — plus several other significant ones.

The collection of risks is richer and growing faster than society, and even science itself, can fully digest. So the cards are being revealed gradually. And not all of them.

At least three critical factors that play an important role in the development of dementia are unlikely to be officially published at the level of the international commission.

Although they are no secret: numerous studies and comments from some of the most authoritative scientists in the field of neurodegenerative diseases have long spoken about them openly.

So, today we have 14 officially “recognized” modifiable risk factors for dementia — that is, factors we can influence. And there will be more. There is little doubt about that.

Scientists state that active prevention may reduce the risk of neurodegeneration by half.

So first, let’s look at each one separately.

1 Risk factor

Low Level of Education

Cognitive Reserve
What does it mean?

Low cognitive reserve means that the brain has a limited ability to compensate for damaged neurons, their number, and their connections. This makes the brain more vulnerable to pathological changes.

Why does it happen?

Low cognitive reserve develops when a person does not receive enough varied and stimulating cognitive load throughout life. This includes a lack of complex intellectual challenges, a low level of education, and no regular habit of learning or developing new skills.

How can this lead to dementia?

The brain is a plastic organ. Cognitive reserves developed through learning and education help compensate for the loss of neurons and their connections.

In people with a higher level of education, or with a habit of learning new things, the brain can use alternative neural pathways to perform tasks.

Depression

Chronic Stress
What does it mean?

Depression is a mental disorder characterized by persistently low mood, loss of interest in life, and reduced activity.

Why does it happen?

Depression can develop for different reasons, including genetic predisposition, hormonal and neurotransmitter system imbalances, chronic stress, and traumatic events.

How can this lead to dementia?

Long-term depression is associated with changes in the brain, including reduced levels of serotonin and other neurotransmitters that play an important role in maintaining cognitive function.

Chronic depression can also contribute to a decrease in hippocampal volume.

3 Risk factor

Obesity

What does it mean?

Obesity is a condition in which excess fat tissue accumulates in the body and can negatively affect overall health.

Why does it happen?

The main causes of obesity include overeating, low physical activity, genetic predisposition, and metabolic disorders.

Unhealthy eating habits, including high consumption of sugar and saturated fats, also play an important role.

How can this lead to dementia?

Obesity promotes inflammatory processes in the body, which can negatively affect the brain.

Metabolic disorders caused by obesity worsen the condition of the vascular system and disrupt the nutrition of neurons.

Excess weight is also associated with insulin resistance, an important factor in the development of Alzheimer’s disease.

Hyperlipidemia in the Form of High LDL

What does it mean?

This is a condition in which there is an excessive concentration of low-density lipoproteins in the blood — LDL cholesterol.

LDL is often called “bad” cholesterol.

Why does it happen?

The main causes of hyperlipidemia include poor diet, obesity, low physical activity, and genetic predisposition.

Eating foods rich in saturated fats and trans fats contributes to higher cholesterol levels.

How can this lead to dementia?

High cholesterol can lead to the formation of atherosclerotic plaques, which narrow the arteries and worsen blood supply to the brain, contributing to the development of vascular dementia.

In addition, hyperlipidemia causes chronic inflammation, which also increases the risk of neurodegenerative diseases.

Physical Inactivity

Lack of Regular Movement
What does it mean?

Physical inactivity means a lack of regular physical activity. It weakens the immune system and the functioning of internal body systems, and increases health risks — from metabolic disorders to cognitive problems.

Why does it happen?

The main causes of physical inactivity include a sedentary lifestyle, lack of time, laziness, lack of motivation and knowledge about its importance, and chronic diseases that limit mobility.

How can this lead to dementia?

Physical activity stimulates the production of growth factors, including BDNF, which support neuroplasticity.

It also improves blood circulation and helps maintain cardiovascular health, which is directly connected to brain health.

A lack of physical activity weakens these processes, increasing the risk of cognitive impairment and dementia.

6 Risk factor

Diabetes

What does it mean?

Diabetes is a chronic disease in which blood glucose regulation is disrupted, leading to elevated blood sugar levels.

Why does it happen?

Diabetes can develop because the pancreas does not produce enough insulin, or because the body’s cells cannot use insulin properly.

Causes include genetic predisposition, obesity, poor diet, and lack of physical activity.

How can this lead to dementia?

Elevated blood glucose damages blood vessels, including vessels in the brain, which worsens the brain’s nutrition.

Insulin resistance, typical of type 2 diabetes, negatively affects brain metabolism.

This contributes to inflammatory processes and oxidative stress, which are risk factors for Alzheimer’s disease.

7 Risk factor

Smoking

What does it mean?

Smoking means inhaling tobacco smoke saturated with nicotine and many other toxic substances formed during combustion.

Why does it happen?

Smoking most often begins because of social factors, stress, or imitation of others.

Over time, addiction develops due to the effect of nicotine on the brain’s neurotransmitter systems.

How can this lead to dementia?

Toxins contained in tobacco damage blood vessels and contribute to inflammatory processes, worsening blood supply to the brain.

Nicotine causes blood vessels to narrow, reducing the flow of oxygen and nutrients to neurons.

Carbon monoxide in cigarette smoke lowers oxygen levels in the blood, worsening brain hypoxia.

All of this can lead to accelerated neuronal degradation and cognitive decline.

Alcohol Consumption Above the Safe Limit

What does it mean?

Excessive alcohol consumption means drinking alcohol in amounts that exceed safe health limits, leading to acute or chronic toxic effects on the body.

Why does it happen?

It can develop because of genetic predisposition, stress, social influence, and other factors.

The body becomes used to alcohol, and more is needed to achieve the familiar effect.

How can this lead to dementia?

Alcohol is toxic to neurons and contributes to their destruction.

Chronic alcohol abuse leads to brain atrophy, especially in areas responsible for memory and cognitive functions, such as the hippocampus and frontal lobes.

Alcohol disrupts the balance of neurotransmitters and causes vitamin B1, or thiamine, deficiency, which contributes to cognitive impairment.

Air Pollution

What does it mean?

Air pollution is the presence of harmful substances in the atmosphere, such as fine particles, nitrogen oxides, sulfur dioxide, and other toxins that damage health.

Why does it happen?

The main sources of air pollution are vehicle exhaust, industrial emissions, burning of fossil fuels, agricultural activity, waste burning, construction and road work, household appliances, heating systems, and other sources.

How can this lead to dementia?

Polluted air contains microparticles that can enter the body through the respiratory system and reach the bloodstream.

These particles trigger inflammation and oxidative stress, which damage brain cells.

Chronic exposure to polluted air increases the risk of cognitive impairment and raises the risk of developing dementia.

Hypertension

What does it mean?

Hypertension is elevated blood pressure that damages blood vessels over time.

Why does it happen?

Hypertension can be caused by genetic predisposition, an unhealthy lifestyle, lack of physical activity, and poor diet.

How can this lead to dementia?

High blood pressure damages small blood vessels in the brain, worsening blood supply and nutrition for neurons.

Chronic hypertension can lead to microstrokes and atrophy of the brain’s white matter, contributing to the development of vascular dementia.

Over time, blood vessels lose elasticity, which disrupts normal blood circulation in the brain and reduces cognitive function.

Constant strain on the vessels also triggers inflammatory processes that contribute to the death of neurons.

11 Risk factor

Vision Problems

What does it mean?

Vision problems include conditions such as cataracts, glaucoma, age-related macular degeneration, and others that can impair the ability to see and process visual information.

Why does it happen?

Vision problems can arise for different reasons, including age-related changes, genetic predisposition, chronic diseases such as diabetes, and external factors such as ultraviolet radiation.

How can this lead to dementia?

Significant decline in visual acuity limits a person’s ability to interact fully with the surrounding world.

This, in turn, leads to reduced cognitive stimulation, increased isolation, and higher stress levels.

A lack of visual information increases the load on other sensory systems and cognitive functions, which over time may contribute to cognitive decline.

Partial or Complete Hearing Loss

What does it mean?

Hearing loss is a reduced ability to perceive sound information, making communication and perception of the surrounding environment more difficult.

Why does it happen?

The main causes of hearing loss include injuries, age-related changes, long-term noise exposure, such as working in a noisy environment or listening to loud music, genetic predisposition, and infections.

Diabetes and cardiovascular diseases can also contribute by worsening blood supply to the hearing organs.

How can this lead to dementia?

Hearing loss directly affects cognitive health by contributing to social isolation, depression, and reduced cognitive stimulation.

In addition, the brain is forced to redistribute its resources, increasing the load required to process sounds.

This reduces available capacity for other cognitive functions, such as memory and thinking, and can accelerate cognitive impairment.

Head Injuries

What does it mean?

Head injuries are physical damage to brain tissue caused by blows, falls, or other external impacts.

Why does it happen?

Head injuries can be caused by accidents, contact sports, crashes, or violence.

How can this lead to dementia?

Head injuries, especially repeated ones, increase the risk of chronic traumatic encephalopathy and Alzheimer’s disease.

Damage to brain tissue can trigger inflammatory processes, disrupt the integrity of neural connections, and reduce neuroplasticity, eventually leading to cognitive impairment.

Social Isolation

What does it mean?

Social isolation is a lack of regular interaction with other people and insufficient social support.

Why does it happen?

Social isolation may be caused by the loss of loved ones, declining health, retirement, lack of motivation for social activity, or physical limitations.

How can this lead to dementia?

A lack of social activity reduces the level of cognitive stimulation needed to maintain brain health.

This is an important aspect of mental activity that stimulates neuroplasticity and supports cognitive function.

Social isolation also increases stress levels and the likelihood of depression, which in turn raises the risk of cognitive impairment and dementia.

Someone may need this
